PHYSICAL ACTIVITY REFERRAL SCHEME
The Healthwise physical activity referral scheme introduces individuals to the benefits of physical activity in order to help reduce risk factors for chronic disease and manage existing medical conditions.
How to register?
If you are interested in Healthwise, please speak to your GP or Practice Nurse, who will review your request. Once approved, your GP or Practice Nurse will send your referral form to us, and we will contact you to book your first appointment.
How much does it cost?
Whether you’re interested in visiting a few times a month or looking for all-inclusive access to activities more than once a week, you’ll find reduced-price pay-as-you-go or monthly Direct Debit options that work for you.

PHASE IV CARDIAC CLASS – Coming soon
Phase IV Cardiac gym classes have been developed for graduates who have completed a Phase III programme and for individuals with established Coronary Heart Disease referred from their GP. Phase IV runs over 12 weeks. Classes are supervised by our fully qualified Cardiac Rehab Specialist Exercise Instructors.
How to register?
Following completion of the Phase III Cardiac Rehabilitation programme, you can be referred to continue into our Phase IV programme. You can also be assessed and referred by your GP. Once the referral is received, we will contact you to book your first appointment.

Now, I go regularly to the Sleaford Gym on my own and feel confident, at last, that I belong there.
Finally, this 12 week program has been an eye opener to me and the first step in my fitness journey. I have seen my body shape change, improvements in the strength of my limbs and seen my weights increase at the gym. I have signed up as a member for the gym and I actually miss the gym if I don’t go at least twice a week. “
